What we collect
Through the contact forms: name, email, phone (optional), contact preference, language and an optional short message. Through the professionals’ form: name, profession, institution (optional), email, phone (optional) and an optional short message. We expressly ask you not to send detailed clinical information or patient-identifying data through these forms.
Purposes
To reply to your request and book the assessment consultation, or to reply to a professional contact. We do not use this data for marketing.
Legal basis
Your consent, given when you submit the form, and steps taken at your request before any contract. You can withdraw consent at any time.
Who processes data on our behalf
Forms are processed by Netlify, Inc., this website’s hosting provider, acting as processor. Netlify may process data outside the European Union, with the safeguards provided by the General Data Protection Regulation (for example, standard contractual clauses).
How long we keep it
For as long as needed to reply. If no clinical relationship is established, the data is deleted within 12 months at most. Patients’ clinical information follows the specific rules for medical records.
Your rights
You can request access, rectification, erasure, restriction or portability of your data and object to processing by writing to info@ketamed.pt. You also have the right to lodge a complaint with the Portuguese data protection authority, CNPD (www.cnpd.pt).
